Improving Access to Audiology Services in England - Department of Health document is due to be published this week and will be available on the following website www.18weeks.nhs.uk

Two teams of Audiologists were finalists in the 2006 Health and Social Care Awards and attended the Awards Ceremony in London on 26th February. A team from the MRC Hearing and Communications Group led by Professor Adrian Davis were finalists in the Innovative Technology Section for developing a hand-held screener manufactured by Siemens that can be used in primary care for screening the hearing of patients before referral to an audiology department. The team from the Audiology Department at Buckinghamshire Hospitals NHS Trust were finalists in the Improving Access category for their work on reducing waiting times for audiology treatment.

Health Select Committee have called for oral evidence from 3 representatives - Audiology, Charing Cross Hospital, RNID and a representative of the PPP companies. Its on 8 March at 10-15 in Portcullis House, London. Pauline Beesley is attending on behalf of BAA and will feedback to members.

The BAA Board were informed yesterday that Prof Stuart Gatehouse sadly passed away at the weekend. All members of the BAA would like to extend their condolences to Stuart's family and colleagues and that our thoughts are with them at this time of sadness

A letter has gone out to NHS SHA CEs Re: Adult Hearing Services from Duncan Selbie (Commissioning Director NHS). The letter is NHS Gateway reference no 7542 and requests PCT commissioning proposals. It states "PCTs need to commission sufficient activity to reduce waits very substantially for audiology and hearing aid fitting so that referrals do not divert onto 18 week pathways."

BSA Council express Members’ concern regarding 18-week patient pathway strategy.

300k Episodes England IMPORTANT - Heads of Department - It has come to BAA's attention that Strategic Health Authorities are in the process of being asked for and submitting numbers to the Department of Health for what is required locally from the 300K episodes announced by Lord Warner. It could be that as for the ISTC contract numbers you are not being involved in the decision about local numbers. We urge you strongly to get in touch with your local contacts and make sure that you have input to this decision. If similar numbers are chosen to the ISTC contracts it could be that your core NHS service has no activity left. If you do get in touch with your SHA and it appears that the numbers commissioned through the IS contracts will detrimentally effect your core activity then please email Jonathan Parsons President of BAA and he will liaise with the Department of Health.

Audiology Graduate Wins Prestigious D W Robinson Prize

Have you considered standing for the BAA board? As part of the final transition arrangements following amalgamation this is the last occasion in which six members of the current board must step down, although some may re-apply. It is important that we attract some enthusiastic individuals to take forward the work of BAA. I would urge anyone who thinks that they may wish to apply to give it proper consideration. It is an honour to be party to and have influence over decisions that are taken for the furtherance of our profession, but it is also a development opportunity that will give you exposure to different ways of working and management structures. Hearing Aids make leader letter to the Times here

Confirmation from DH with regard to Hearing Aid funding: A letter went out to SHA Directors of Finance on 26 July regarding the central budget bundle allocation. With it went a draft service level agreement that set out the required outputs from the funding (also went to SHA CEs). For Older People and Disability one of the 'Objectives and deliverables' was that the money will: 'Contribute to the delivery of audiology and digital hearing aid services within the 18 week and diagnostic access targets'. There is no direction on exactly how the local allocations/priorities are determined.

Re - FAQS - ADULT HEARING SERVICES (AHS) AND 18 WEEKS I have to say that I am very disappointed - this does absolutely nothing to alleviate the current difficulty over commissioning created by the 18 week announcement. BAA have started to collect the data from sites and exactly as I have described to you, local commissioners, pressured to establish financial balance have viewed this as an opportunity not to invest to even the MHAS levels for hearing aid services. No target - no investment. There is still no clarity in the definitions about what is supposed to be collected and what is not. Are we correct to understand from the FAQ 4, that the assessment associated with a hearing aid assessment does count against the interim targets, but that the provision of a hearing aid does not? Is it not easy to see that the implications of this will be that patients are assessed for a hearing aid and that the waiting list will be cleared of assessments, but that the patient will have little prospect of getting a hearing aid? By the time they are seen for dispensing of an aid, the assessment will be out of date and need to be repeated. Is this not a monumental waste of time and money? It is not just Adult Hearing Services - it would be helpful to set out what counts in relation to paediatric audiology for example - what about a PTA as part of an paediatric audiology assessment. This seems to have been an opportunity missed for clarity over what is to be counted as well as to help with commissioning. We note also that we have not received replies to either of the two letters written to DH on this matter. I have copied this email to all BAA members via the web site. There is a large and increasing intolerance from BAA members of the lack of strategic vision and coherent direction for audiology services. We need swift action - mention of a national action plan, without any concrete timescale continues to leave the service in limbo without any driver for investment.

18- week update from the President We have not as yet received a reply to either letter, but I spoke to Sue Hill at the end of last week and we are promised a reply to both. The Department of Health (DH) have compiled a list of questions (BAA has had input) in relation to Audiology and the 18 week targets in England and they should be on the 18 week website this week. We will point out when this has happened and give an analysis of how we see the situation following sight of the answers. It is evident that 18 weeks and the non 'ringfencing' of MHAS money presents very significant problem for some services in maintaining capacity. BAA need to know about the scale of the problem in order to best represent you. We have set up a confidential email address for you to let us know where services are having difficulties. This will be completely confidential and we will not name any sites in our discussions with DH. If we do not know how many services are experiencing problems DH will assume that there is not a significant problem. We have also reopened the 18-week discussion area on the website and will welcome comments and discussion.

18 week wait hot news. BAA are extremely frustrated and angered that our advice and that of other organisations has been ignored. There is the potential for considerable disruption of the modernised service models that we have been so effective in delivering. More than anything it appears to be a betrayal of the large numbers of patients on our waiting lists. My personal fear is that at this time of threatened budgets and the possible non materialisation of the MHAS funds that we were told were devolved to local budgets, we now lack the driver that will empower discussions with commissioners. Members of the BAA board strongly expressed our dissatisfaction with the situation at an 18 week development site meeting yesterday and the following letter was sent to Professor Sue Hill, Chief Scientific Officer, this morning.

Following a recent BSA mail shot which included an advert for courses from Seabrook Audiology, BAA have responded to unease from members by agreeing the following statement with BSA: It is important to set these courses in the context of the new skills escalator for audiology as they are aimed at for example ATOs and Teachers of the Deaf. Although some elements of the skills escalator are still being discussed, and the exact roles are yet to be determined, we may expect that impression taking for example will be a level 4 task, the province of the Associate Audiologist, and that this will be a foundation degree or Dip HE qualification. These courses provide valuable introductions, but at this time we are unable to confirm what credits may be given for particular modules and how they may contribute to the portfolio of a band 3 (Assistant) practitioner wishing to become a band 4 - Associate. Business Planning: As we get into the Autumn and the LDP round of the business planning cycle (England) it is worth alerting members that the RNID will be running a campaign in the near future targeting patients (UK wide) with hearing loss who will not have traditionally accessed the service. RNID have promised to liaise with BAA in terms of alerting us to proposed publicity, so you can anticipate further updates. The point is that this may well increase demand over and above what you have expected in terms of any waiting list targets and it is worth putting down a marker in LDP submissions to this effect.

Following the recent MAWFET (Workforce Planning) Meeting arrangements are expected soon for further meetings:
* DOH/BAA and interested parties on the future contribution of practitioners below the degree level in the skills escalator. BAA have started work on populating the skills escalator with National Occupational Standards and Education and Training pathways, but DOH has embargoed circulation
* DOH/BAA/BSHAA/HAC - registration issues.
Further information will be posted when available.

In addition to previous postings about the awaited update from DOH in relation to registration and acceptable qualifications / routes in the short term, the latest information is that this is perhaps a further two weeks away. There appears to be a complex series of negotiations occurring between DOH / HPC / solicitors etc. Rest assured that BAA continue to stress the urgency of this and push for the earliest response.
 

The first Professional Development committee meeting will be on 23rd August in Liverpool and will focus on CPD schemes for BAA members and accreditation of post-registration training.

Further to the information posted on 17/05, the meeting with Sue Hill has now taken place. It is safe to say that there is an emerging view in relation to the mechanics of the RCCP transfer to the Health Professions Council and how this will be accommodated. There will be an announcement from DOH in the very near future once Sue has consulted with HPC, WDCs and the home country devolved institutions. This will be available on the BAA website as soon as it is received.

News.. After repeated calls for reaction to the crisis caused by the low numbers of trained audiologists and those in commissioned training places, it may just be possible that the old traditional routes, or a version of them could be acceptable for those starting training this year. BAA acknowledges that this gives commissioners and planners of training places very little time to react, but have been governed by DOH in relation to timescales. Jonathan Parsons has a meeting on 27th May with Sue Hill, CSO and RCCP where hopefully a clear way forward will be explicit. This will be articulated via the website as soon as a decision has been made. You may wish to make WDCs etc aware that there may be a last minute change.
